Although it may appear to be a minor localised infection, if not treated in time it can lead to painful complications and chronic recurrences. Identifying it quickly and acting decisively is key to avoiding sequelae such as anal fistulas.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>What is a perianal abscess and why does it form?<\/strong><\/h3>\n\n\n\n<p>A perianal abscess is a <strong>accumulation of pus in the tissues near the anus and rectum<\/strong>The infection is caused by an infection in the anal glands leading to the anal canal.<\/p>\n\n\n\n<p>These normally inconspicuous glands can become clogged and give rise to a small pocket of infection. If the bacteria multiply, the infection progresses and a purulent collection forms. This causes pain, redness and swelling in the area, and can make it difficult to perform such basic functions as sitting, walking or going to the toilet.<\/p>\n\n\n\n<p>Risk factors:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Chronic constipation or excessive straining.<br><br><\/li>\n\n\n\n<li>Inflammatory bowel disease (especially Crohn's disease).<br><br><\/li>\n\n\n\n<li>Diabetes mellitus.<br><br><\/li>\n\n\n\n<li>Trauma, microcracks or previous interventions in the area.<br><br><\/li>\n\n\n\n<li>Immunosuppression.<br><br><\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>Main symptoms of perianal abscess<\/strong><\/h3>\n\n\n\n<p>Symptoms develop in the form of <strong>fast and intense<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Acute and throbbing pain<\/strong> in the anal area, which is worse when sitting, walking or defecating.<br><br><\/li>\n\n\n\n<li><strong>Visible inflammation<\/strong> in the form of a hard, red, warm lump near the anus.<br><br><\/li>\n\n\n\n<li><strong>Fever<\/strong> and general malaise (in more advanced cases).<br><br><\/li>\n\n\n\n<li><strong>Difficulty or fear of defecation<\/strong> by pain.<br><br><\/li>\n\n\n\n<li>In some cases, <strong>spontaneous discharge<\/strong> of pus if the abscess drains on its own.<br><br><\/li>\n<\/ul>\n\n\n\n<p>The presence of these signs is reason enough for consultation <strong>urgently<\/strong> with a specialist in general surgery or proctology.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>What is the difference between an abscess and an anal fistula?<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The <strong>perianal abscess<\/strong> is an acute, localised infection. It may resolve completely if properly drained.<br><br><\/li>\n\n\n\n<li>The <strong>anal fistula<\/strong> is a <strong>chronic complication<\/strong> which occurs when, after the abscess, a small duct forms that connects the inside of the anus to the perianal skin. Pus or fluid may continue to drain out of this passage over time.<br><br><\/li>\n<\/ul>\n\n\n\n<p>It is estimated that <strong>up to 50% of abscesses<\/strong> end up generating a fistula if they are not treated correctly or if there are predisposing factors.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>Treatment: antibiotic or surgery?<\/strong><\/h3>\n\n\n\n<p>Although some mild cases may be accompanied by antibiotic treatment, <strong>the majority of perianal abscesses require urgent surgical drainage.<\/strong>. The surgery consists of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Make a small incision over the abscessed area to drain the pus.<br><br><\/li>\n\n\n\n<li>Cleaning of the infected area and occasional placement of a gauze pad (wick).<br><br><\/li>\n\n\n\n<li>Sometimes the wound is left open to allow progressive drainage.<br><br><\/li>\n<\/ul>\n\n\n\n<p><strong>It is a short, safe, outpatient procedure in most cases.<\/strong> Pain relief is immediate after evacuation of the pus.<\/p>\n\n\n\n<p>Exclusive use of antibiotics <strong>is often not enough<\/strong>except in immunosuppressed patients, with extensive cellulitis or high fever without a well-defined collection.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>When is scheduled surgery required?<\/strong><\/h3>\n\n\n\n<p>Once the acute infection has been controlled, if the patient has persistent symptoms or a secondary anal fistula develops, elective surgery may be necessary to resolve the root of the problem.<\/p>\n\n\n\n<p>Indications for planned surgery:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Persistent discharge from an opening in the skin.<br><br><\/li>\n\n\n\n<li>Recurrence of the abscess in the same area.<br><br><\/li>\n\n\n\n<li>Diagnosis of anal fistula by examination or endoanal ultrasound.<br><br><\/li>\n\n\n\n<li>Complex fistulae (curves, multiple paths, sphincter relationship).<br><br><\/li>\n<\/ul>\n\n\n\n<p>Surgical techniques vary according to the type of fistula: fistulotomy, placement of fishing lines, LIFT, flaps, etc. The aim is to heal the fistula without damaging continence.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>Conclusion<\/strong><\/h3>\n\n\n\n<p>A perianal abscess is more than a painful \"pimple\": it is an emergency that requires prompt medical assessment.
