It occurs when, through an old surgical scar, a defect forms in the abdominal wall that allows internal contents (such as fat or intestine) to protrude. Although it may not initially cause discomfort, <strong>its progression can lead to pain, entrapment or even urgent surgery.<\/strong> if not treated in time.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>What is an eventration and how does it form?<\/strong><\/h3>\n\n\n\n<p>An eventration is a hernia that appears <strong>at a previous surgical incision site<\/strong>usually in the abdomen. It is caused by a failure of the muscle wall to heal, allowing some of the intra-abdominal contents to push out through the defect.<\/p>\n\n\n\n<p>This failure may be due to multiple factors that hinder proper tissue repair during the postoperative period.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>Risk factors<\/strong><\/h3>\n\n\n\n<p>Some conditions increase the risk of developing an eventration after abdominal surgery:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Surgical wound infection<\/strong>.<br><br><\/li>\n\n\n\n<li><strong>Obesity<\/strong> or sudden weight gain.<br><br><\/li>\n\n\n\n<li><strong>Intense physical exertion<\/strong> after surgery.<br><br><\/li>\n\n\n\n<li><strong>Chronic cough or severe constipation<\/strong>which increase intra-abdominal pressure.<br><br><\/li>\n\n\n\n<li><strong>Malnutrition<\/strong> or protein deficiency, which hinders healing.<br><br><\/li>\n\n\n\n<li><strong>Multiple surgeries<\/strong> in the same area.<br><br><\/li>\n\n\n\n<li><strong>Chronic smoking<\/strong>.<br><br><\/li>\n\n\n\n<li><strong>Advanced age<\/strong>.<br><br><\/li>\n<\/ul>\n\n\n\n<p>The type of surgical technique used and the way the wound is closed also play a role.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>What are the symptoms?<\/strong><\/h3>\n\n\n\n<p>The main sign of an eventration is the <strong>appearance of a soft lump<\/strong> in the area of an old scar, especially when coughing, straining or standing up. There may be:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Mild to moderate pain<\/strong>especially at the end of the day.<br><br><\/li>\n\n\n\n<li><strong>Feeling of heaviness<\/strong> abdominal pain or discomfort when bending over.<br><br><\/li>\n\n\n\n<li><strong>Spontaneous reduction<\/strong> of the lump when lying down (in early stages).<br><br><\/li>\n<\/ul>\n\n\n\n<p>When the hernia becomes large or complicated, it may cause:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Severe and continuous pain<\/strong>.<br><br><\/li>\n\n\n\n<li><strong>Reddening of the skin<\/strong>.<br><br><\/li>\n\n\n\n<li><strong>Difficulty in reducing the bulge<\/strong> (sign of incarceration).<br><br><\/li>\n\n\n\n<li><strong>Vomiting and symptoms of intestinal obstruction<\/strong>if there is strangulation.<br><br><\/li>\n<\/ul>\n\n\n\n<p>In these cases, it is a surgical emergency.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>How is it diagnosed?<\/strong><\/h3>\n\n\n\n<p>Diagnosis is usually clinical (by physical examination), but in doubtful cases or for planning surgery, it may be required:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Abdominal wall ultrasound<\/strong>.<br><br><\/li>\n\n\n\n<li><strong>Tomography (CT)<\/strong>to measure the size of the defect and assess the herniated content.<br><br><\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>Is it necessary to operate on all eventrations?<\/strong><\/h3>\n\n\n\n<p>Not all, but yes <strong>the majority<\/strong>. In general, surgery is recommended if:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The hernia causes persistent discomfort.<br><br><\/li>\n\n\n\n<li>It progressively increases in size.<br><br><\/li>\n\n\n\n<li>There is a risk of incarceration or strangulation.<br><br><\/li>\n\n\n\n<li>Compromises the patient's quality of life or functionality.<br><br><\/li>\n<\/ul>\n\n\n\n<p>In cases of low risk, small size and very fragile patients, conservative treatment with follow-up and abdominal bracing can be considered, although it is not a definitive solution.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a><\/a><strong>What does the surgery consist of?<\/strong><\/h3>\n\n\n\n<p>The aim is to <strong>repairing the defect in the abdominal wall<\/strong> and, in most cases, to reinforce it with a <strong>prosthetic mesh<\/strong> to reduce the risk of recurrence.<\/p>\n\n\n\n<p>There are various techniques depending on size and location:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Traditional open surgery<\/strong>.<br><br><\/li>\n\n\n\n<li><strong>Laparoscopy<\/strong>Less invasive, less postoperative pain and faster recovery.<br><br><\/li>\n\n\n\n<li><strong>Hybrid or robotic techniques<\/strong> in specialised centres.<br><br><\/li>\n<\/ul>\n\n\n\n<p>The choice depends on the size of the hernia, the patient's general condition and the experience of the surgical team.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><\/h3>\n\n\n\n<p>An eventration is not just a cosmetic problem. If left untreated, it can become enlarged, painful or even life-threatening if the bowel becomes trapped.
